We are accepting workers compensation patients and cash pay patients.
We accept and participate with almost all workers compensation insurance carriers including all of Florida and most out-of-state workers comp. We are YOUR workers' comp specialists here in southwest Florida and we have over 24 years of experience in helping injured workers return to work and have a better quality of life.

Once we receive your completed new patient paperwork, one of our patient coordinators will review your information, verify your insurance, and contact you to make an appointment to see the doctor.
For Workers' Compensation patients you will need to bring to your appointment:
- Your driver's license or any state issued ID that is current and valid.
- Any medications that you are current prescribed including the medication bottles.
- Any additional medical records that you may have.
Now accepting new cash pay (self pay) patients. No insurance, no problem!
As of February 2020 we are now accepting new cash pay (self pay/direct pay) patients for pain management. If you have no insurance or Medicaid, we can accept you as a cash pay (self pay) patient. Dr. Schultz has over 24 years experience treating pain and is board certified in physical medicine and rehabilitation. Along with medication management, Dr. Schultz will provide physical/massage/chiropractic therapy referrals, TENS units, braces, and traction devices.
